This form is intended for use by PSU technical support to request a Static IP for a device such as a server, camera, etc. Fields in bold are required. For questions, contact the Helpdesk at 5-4357 (5-HELP) or help@pdx.edu
For static IP printer requests, use the following form: http://www.uss.pdx.edu/form/printqueue-request.php
Note -- Do not manually set the the static IP on the device. Static IPs are selected and assigned by Networking.
